Ecoantibiotics: a new strategy for increasing the effectiveness of anti-helicobacter therapy and prevention of antibiotic-associated diarrhea

H. pylori is the primary etiological factor in the development of peptic ulcer disease and the leading pathogenetic mechanism of this condition. It causes damage to the mucosal epithelium, reduces its resistance to other aggressive factors, initiates an active inflammatory process in the mucosa, and enhances acid and pepsin production in the gastric glands.
Anti-Helicobacter Therapy
In our country, the infection rate of the adult population with H. pylori is 80%. For people infected with H. pylori, the risk of developing peptic ulcer disease is 10–20%, and the risk of gastric cancer (adenocarcinoma and MALT lymphoma) is 1–2% [1].
Anti-Helicobacter therapy is the primary standard for treating Helicobacter-associated diseases of the gastroduodenal zone, as reflected in international consensus reports (the Maastricht Consensus Reports 1–3, from 1996, 2000, and 2005, respectively). After successful eradication therapy, disease recurrence is noted in only 10–15% of patients. In contrast, when using only antisecretory drugs, which also promote relatively rapid ulcer healing, disease recurrence within the first year after the end of therapy is observed in approximately 70–80% of patients [1, 2].
Virtually all modern anti-Helicobacter therapy regimens are based on the use of antibacterial drugs and proton pump inhibitors (PPIs). The goal of treatment is the complete eradication of both vegetative and coccoid forms of H. pylori in the mucosa of the gastroduodenal zone (Table 1).
